### About the Role
RASAK is seeking a compassionate, reliable and motivated **Community Worker** to join our growing community services team.
The successful candidate will work directly with individuals and families to assess their needs, provide practical support, connect them with appropriate community services and assist them in improving their independence, wellbeing and participation in the community.
### Key Responsibilities
• Assess clients' needs and identify appropriate community support services and resources.
• Provide information, guidance and practical assistance to clients and their families.
• Assist clients to access health, housing, education, employment and community services.
• Develop and implement appropriate support and assistance plans based on individual needs.
• Support clients in developing independence and participating in community activities.
• Liaise with community organisations, service providers and other relevant professionals.
• Advocate for clients and assist them in accessing appropriate services and entitlements.
• Monitor clients' progress and review support arrangements as required.
• Maintain accurate client records, case notes and other required documentation.
• Participate in case meetings and coordinate services with internal and external stakeholders.
• Identify community needs and contribute to the development and delivery of appropriate programs and services.
• Maintain professional boundaries, confidentiality and compliance with organisational policies and relevant legislation.
### About You
The ideal candidate will have:
• Relevant qualifications in Community Services, Social Services or a related discipline.
• Previous experience in community services, case support or a related role.
• Strong communication and interpersonal skills.
• Ability to work effectively with people from diverse cultural and social backgrounds.
• Strong organisational, documentation and case-management skills.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Current Australian driver's licence is desirable.
• National Police Check and Working With Children Check, where required for the role.
• NDIS Worker Screening Check where the position involves relevant NDIS work.
