Angels Care Pty Ltd is a registered NDIS provider headquartered in Marsden Park, New South Wales, delivering person-centred disability support services to participants across Western Sydney and other regions of NSW. Established in 2019, Angels Care provides a comprehensive range of NDIS supports designed to promote participant independence, dignity, choice, community connection and quality of life.

The organisation provides services including assistance with daily living and personal activities, community participation, development of life skills, transport and travel assistance, Supported Independent Living (SIL), accommodation and tenancy supports, support coordination, employment-related assistance, community nursing, therapy supports and other specialised disability supports within its registered scope.

Angels Care follows a participant-first approach, with individual support arrangements developed around each participant’s goals, preferences and circumstances. The organisation works collaboratively with participants, families, carers, support coordinators, health professionals and community organisations to deliver coordinated and responsive support. Its service model emphasises person-centred care, dignity and respect, participant choice and control, inclusion, accountability and continuous improvement.

As Angels Care continues to provide disability and community-based supports across NSW, the organisation requires an experienced Community Worker to assist participants in accessing appropriate services, developing independent living and social skills, strengthening community connections and working toward their individual goals.

About the Role

The Community Worker will be responsible for assessing participants’ individual needs, assisting in the development and implementation of appropriate support strategies, facilitating access to community services and resources, monitoring participant progress, and supporting people living with disability to increase their independence and participation in community life.

Working collaboratively with participants, families, carers, support coordinators, allied health professionals, healthcare providers and community organisations, the Community Worker will contribute to coordinated and person-centred service delivery.

The role will have a particular focus on community participation, capacity building, life skills development, participant advocacy and access to appropriate community resources, consistent with Angels Care’s participant-first approach and NDIS service model.

Key Responsibilities

• Conduct participant interviews and needs assessments to identify individual circumstances, goals, strengths, barriers, risks and support requirements.

• Assist in developing, implementing, monitoring and reviewing individualised support strategies consistent with participants’ NDIS plans, goals and preferences.

• Support participants in developing practical independent living skills, interpersonal skills, confidence and capacity to participate meaningfully in their communities.

• Assist participants to identify and access appropriate community resources and services, including healthcare, allied health, housing, education, employment, transport and recreational opportunities.

• Facilitate participant involvement in community, social and recreational activities that promote inclusion, independence and meaningful community connections.

• Support participants through important life transitions and assist them in developing skills required for greater independence and participation.

• Monitor participant wellbeing and progress, evaluate support outcomes, maintain accurate case notes and prepare appropriate progress and service reports.

• Provide participants and their families with information, guidance, advocacy and practical assistance regarding available disability and community support services.

• Liaise with families, carers, support coordinators, allied health professionals, healthcare providers, government agencies and community organisations to facilitate coordinated service delivery.

• Identify social, environmental or practical barriers affecting participant independence and assist in developing appropriate strategies to address those barriers.

• Encourage participants to exercise choice and control over their supports and actively participate in decisions affecting their lives.

• Maintain accurate and confidential participant records, case management documentation, incident records and service reports in accordance with organisational and NDIS requirements.

• Promote participant dignity, rights, privacy, cultural inclusion, safety and person-centred practice in all aspects of service delivery.

• Participate in participant reviews, multidisciplinary meetings, case discussions and service planning activities where required.

• Contribute to continuous improvement initiatives aimed at strengthening participant outcomes, community participation and service quality.

• Perform duties in accordance with NDIS Practice Standards, the NDIS Code of Conduct, Work Health and Safety requirements, safeguarding obligations and Angels Care policies and procedures.

Requirements

• Australian citizens and permanent residents are encouraged to apply.

• AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Disability, Human Services, Welfare, Individual Support or a closely related field, or relevant experience where permitted for the applicable occupational classification.

• Demonstrated experience in disability services, community services, human services or NDIS-funded programs.

• Sound understanding of person-centred practice, participant choice and control, safeguarding principles and community-based service delivery.

• Knowledge of the NDIS and the range of community and disability support services available to participants.

• Demonstrated ability to assess participant needs and assist in developing practical support strategies.

• Strong communication, interpersonal, advocacy and relationship-building skills.

• Strong organisational, problem-solving, documentation and case coordination abilities.

• Ability to establish professional relationships with participants, families, carers, service providers and community organisations.

• Ability to maintain accurate participant records and prepare clear professional reports.

• Compassionate, respectful, reliable and participant-focused approach to service delivery.

• Ability to work independently while contributing effectively within multidisciplinary and service-delivery teams.

• Current NDIS Worker Screening Check, National Police Check, Working with Children Check where applicable, and First Aid/CPR certification or willingness to obtain.

• Current Australian Driver Licence is desirable.

Work Conditions

• Full-time, ongoing position.

• Standard operational hours with flexibility according to participant and service-delivery requirements.

• Work may be undertaken from the Marsden Park office, participant homes, community settings and other service locations.
