The Client
P2P Care Services is a registered NDIS provider delivering person-centred disability and community support services. The organisation supports people with disability to build independence, exercise choice and control, and participate meaningfully in their communities.
The Role
The Community Worker will assess participant needs, coordinate appropriate services and referrals, liaise with families and service providers, and assist participants to access community programs and resources that support their individual goals, independence and social participation.
This is for a full time role with an annual salary set at $80,000 – $85,000 depending on experience plus superannuation, and will be based in Campbelltown, NSW
The Responsibilities
Assessing participants' needs, circumstances and goals and identifying appropriate community, welfare, disability and social support services.
Developing and coordinating person-centred support arrangements in consultation with participants, families, carers and relevant service providers.
Providing participants with information about available community programs, services and resources and assisting them to access appropriate supports.
Liaising with community organisations, government agencies, health professionals, support coordinators and other service providers to coordinate services and referrals.
Supporting participants to increase independence, social participation and engagement with their local community.
Monitoring participant progress and reviewing support arrangements where needs, circumstances or goals change.
Advocating for participants, where appropriate, to assist them in accessing suitable services, resources and community opportunities.
Identifying barriers to community participation and assisting participants to develop practical strategies to overcome them.
Building and maintaining professional relationships with community organisations and service providers to improve access to appropriate supports.
Maintaining accurate case notes, participant records, referral information and other required documentation.
Preparing reports relating to participant progress, support outcomes and service delivery.
Working collaboratively with participants, families, colleagues and external stakeholders while maintaining confidentiality, professional boundaries and person-centred practice.
Requirements
AQF Diploma or higher in Community Services, Community Development, Welfare, Social Services or a related field.
At least 12 months relevant work experience as a Community Worker or in a similar community services role
At least 12 months relevant work experience of case management, report preparation, and submission processes including NDIS documentation
At least 12 months demonstrated experience working with vulnerable individuals, people with disabilities, or culturally diverse communities
At least 12 months of experience dealing with mental and emotional disturbances and developmental disability
At least 12 months of experience working with government agencies, non-profits, and other service providers
Have a current First Aid (HLTAID003 or HLTAID004) and CPR (HLTAID001) certificate
Current Working with Children Check
Current AFP Police Check
Current NDIS Worker Screening Clearance
Strong communication, stakeholder engagement, advocacy and problem-solving skills.
Knowledge of person-centred practice, confidentiality, safeguarding, WHS and the NDIS environment.
Ability to work independently and collaboratively while maintaining professional boundaries and confidentiality.
