Community Worker

The Role:

As our Community Worker, you will deliver community-based support services and implement programs aimed at improving the social, emotional, and practical wellbeing of individuals and families.

Responsibilities:

• Assess clients’ needs and identify appropriate support services in line with individual circumstances

• Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders

• Provide information, guidance, and referrals to community services, including health, housing, and social support services

• Support clients in accessing community resources, programs, and government services

• Liaise with healthcare providers, social workers, and other service agencies to coordinate care and support outcomes

Skills & Attributes:

• Relevant qualification

• Minimum 1 year of relevant experience

• Strong interpersonal and communication skills

• Ability to work independently and as part of a team

• Demonstrated empathy, patience, and commitment to client wellbeing

• Knowledge of NDIS framework and person-centred care principles (preferred)
