About the Role
We are seeking a dedicated Community Worker to join our client’s team, a trusted provider of person-centred support services for individuals with disabilities, mental health needs, and complex behaviours. In this role, you will empower clients to achieve independence, build life skills, and engage meaningfully with their community. Our client operates across supported independent living, community access, and in-home care settings.
Key Duties and Responsibilities
Client Assessment & Support Planning
• Conduct in-depth interviews to evaluate clients’ physical, emotional, and social challenges, then develop tailored support and educational programs.
• Monitor client progress through regular check-ins and detailed case notes, adjusting plans to meet evolving needs.
Referral & Community Liaison
• Connect clients with specialist agencies—including health, housing, and employment services—to ensure holistic care.
• Build partnerships with community groups, government bodies, and private providers to promote resource awareness and address local issues.
Direct Client Support & Advocacy
• Assist children, adults, and families in group housing or adult service units, focusing on disability care, education, and emotional well-being.
• Guide young people in resolving social, emotional, and financial difficulties, while supervising offenders on probation or parole as required.
Funding & Reporting
• Prepare detailed submissions for funding and resources and compile reports for government bodies to demonstrate program outcomes.
Essential Requirements
• Diploma in Community Services, Social Work, or related field (or equivalent experience).
• 1 year of full-time work experience within community services, social work, or in related positions.
• Proven ability to assess complex needs and implement evidence-based support plans.
• Strong communication skills for liaising with diverse stakeholders, including vulnerable populations.
• Experience with NDIS frameworks, mental health support, or disability services is highly regarded.
• Relevant qualification to above field.
