The Case Manager, Lockyer Valley Housing Service (LVHS), supports individuals and families experiencing or at risk of homelessness through assessment, case management, advocacy, service coordination and referrals. Working across centre-based, outreach and remote service delivery, this role helps clients secure and sustain stable housing while fostering independence and connections to community supports.
Key responsibilities of this role include:
Assessing eligibility, needs and risks of individuals and families experiencing or at risk of homelessness.
Providing holistic case management, including support planning, advocacy and tenancy sustainment assistance.
Coordinating services and referrals to ensure clients can access the support they need.
Build and maintain effective relationships with community partners, service providers and stakeholders.
Delivering a high-quality, customer-focused service that places client needs at the centre of all interactions.
Maintaining accurate case notes, client records, reports and program data in line with organisational requirements.
Working collaboratively within a multidisciplinary team and participate in meetings, supervision and professional development.
Supporting a safe, inclusive and values-driven workplace, including mentoring volunteers and student placements when required.

What We're Looking For
Relevant qualification in Human Services, Social Work, Community Services or a related field, or equivalent sector experience.
Experience in assessment, case management, advocacy and referral pathways, ideally within the homelessness, housing or community services sector.
Strong communication and relationship-building skills, with the ability to engage and support people with complex needs.
Ability to work collaboratively with stakeholders and community networks while managing a varied caseload with a high level of autonomy.
Proficient in Microsoft Office and client management systems, with strong administration, record-keeping and reporting skills.
Essential Requirements
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver's Licence and willingness to travel for work
