Apex Sydney Pty Ltd trading as Eureka Home Care is a Sydney-based provider of personalised aged care, disability support and home and community care services. The organisation provides personal care, domestic assistance, in-home support, social and respite support, transport and community access, as well as allied healthcare and other services designed to help clients maintain their quality of life and independence. Its services are delivered by professionally trained care staff in clients' homes and community settings.

Eureka Home Care is seeking a compassionate and experienced Residential Care Officer to provide direct care, supervision and day-to-day support to clients with aged-care and disability-related needs. The position will involve assessing and responding to individual client needs, implementing care and support plans, assisting with daily living activities and monitoring clients' wellbeing and progress.

The Residential Care Officer will work closely with clients, families, care professionals and other service providers to promote independence, dignity, safety and participation in everyday activities and the wider community. The role will also require accurate documentation, communication of changes in client needs and appropriate escalation of concerns to relevant supervisors or professionals.

Key Responsibilities

- Provide personal care and day-to-day support to clients in accordance with individual care plans, assessed needs and organisational procedures.

- Assist clients to attend medical appointments, shopping centres, social activities and community events and support their participation in community life.

- Observe, monitor and report changes in clients' physical, emotional, behavioural or functional condition and promptly escalate concerns in accordance with care procedures.

- Interview and communicate with clients and, where appropriate, families or carers to understand individual needs, preferences, difficulties and support requirements.

- Assist in identifying client needs and contribute relevant observations to the development and review of individual support and care plans.

- Implement individual care, support, education and activity programs designed to address identified client needs and promote independence.

- Provide companionship, reassurance and emotional support and encourage clients to maintain relationships, interests, routines and social connections.

- Support respite and social activities, including assisted outings, exercise, classes, recreation and other activities consistent with individual interests and care plans.

- Monitor and document clients' progress against established care and support objectives and provide accurate updates to supervisors and relevant care professionals.

- Maintain accurate and confidential records of client care, activities, observations, incidents and other relevant support information.

Qualifications and Experience Required

- AQF Associate Degree, Advanced Diploma or Diploma in Ageing Support, Disability, Community Services, Individual Support, Nursing or a related field.

- At least three years of relevant experience may substitute for the formal qualification requirements.

- Previous experience supporting older people, people with disability, people recovering from illness or surgery, or clients with complex support requirements will be highly regarded.

- Experience working in home care, community care, residential care or disability support environments.

- Current First Aid and CPR certification, or willingness to obtain and maintain required certification.

- Strong communication and interpersonal skills and the ability to develop respectful relationships with clients and their families.

- Ability to understand and implement individual care and support plans accurately.

- Good observation and reporting skills, with the ability to identify and communicate changes in client condition or support needs.

- Understanding of professional boundaries, privacy, confidentiality and respectful care practices.

- Sound knowledge of safe work practices, manual handling, workplace health and safety and infection-control requirements.

- Ability to respond appropriately to challenging situations and seek assistance or escalate concerns when required.
