Social Worker (x2)

Redeemed Care Inc About the Role We are seeking 2 qualified Social Workers to provide professional assessment, case management, advocacy and support to individuals, children and families experiencing disability, psychosocial, family and social challenges. The role involves working collaboratively with clients, families, community organisations and multidisciplinary stakeholders to improve wellbeing, strengthen protective factors, promote independence and connect clients with appropriate services and supports.

Key Responsibilities • Assess clients’ social, emotional and practical needs and develop appropriate support and intervention plans • Provide case management, counselling, advocacy and referral support to individuals and families experiencing vulnerability • Work collaboratively with children, families and carers to strengthen wellbeing, safety and independence • Support clients living with disability to understand and implement relevant support plans, including NDIS-funded supports where applicable • Undertake psychosocial and risk assessments and contribute to safety planning and crisis intervention • Apply strengths-based, trauma-informed, culturally responsive and person-centred social work practice • Advocate for clients in relation to housing, healthcare, education, disability, family and community services • Liaise with government agencies, schools, healthcare providers, allied health professionals and community organisations • Monitor client progress, review support strategies and coordinate multidisciplinary services • Maintain accurate case notes, assessments, reports and other professional documentation • Identify and respond appropriately to child protection, family violence and safeguarding concerns • Promote social inclusion, community participation and development of informal support networks • Contribute to service planning, continuous improvement and compliance with relevant professional and organisational standards Essential Requirements • Diploma, Bachelor degree or higher qualification in Social Work from a recognised tertiary institution • Eligibility for membership of the Australian Association of Social Workers (AASW) • Relevant professional experience in social work, case management, family support, disability or community services • Demonstrated knowledge of child protection, family violence, safeguarding and risk assessment principles • Knowledge of person-centred, strengths-based and trauma-informed practice • Strong assessment, case management, communication, advocacy and report-writing skills • Ability to work effectively with culturally and linguistically diverse clients and multidisciplinary stakeholders • Current National Police Clearance • NDIS Worker Screening Check where required for the role • Working With Children Check (WWCC) • Current First Aid & CPR • Valid WA Driver’s Licence Employment Conditions • Full-time position • Work will be undertaken in accordance with Redeemed Care Inc policies and applicable employment conditions • Applicants must satisfy all mandatory screening, registration and qualification requirements relevant to the position • Applications close 28 days from date of advertisement • Applicants must submit Resume, Cover Letter and copies of qualifications and relevant registrations About Company About Us Redeemed Care Inc is a community-focused organisation delivering high-quality support services to individuals, children and families across Western Australia.
