Experienced Practice Nurse required 1 day/week

Synergy Medical Practice is an independently owned and operated general practice located in the heart of St Leonards on Sydney's Lower North Shore. We are committed to delivering high-quality, patient-centred care and are seeking an experienced Practice Nurse to join our friendly team.

About the Role

We are offering a part-time position of approximately 10 hours per week , with flexibility to split the hours across two days . Some duties may also be performed remotely, providing the opportunity to work from home for part of your contracted hours.

This role would suit a nurse seeking flexibility while remaining engaged in meaningful clinical and preventative healthcare work.

Key Responsibilities

Chronic disease management and care plan coordination

Health assessments and recalls

Immunisations and vaccine management

Patient education and health promotion

Assisting with quality improvement activities

Clinical triage and patient follow-up

Management of recalls and reminders

Support with accreditation and compliance activities

General nursing support to our GPs and clinical team

About You

To be successful in this role, you will have:

Current AHPRA registration as a Registered Nurse

Experience in general practice nursing (preferred)

Strong organisational and communication skills

A patient-focused approach and commitment to high-quality care

Experience with chronic disease management and preventative health programs

Ability to work both independently and collaboratively within a small team

Proficiency with clinical software systems and Microsoft Office applications
