About the Role

An exciting opportunity exists for compassionate and experienced Health Care Workers to join the casual pool at Sister Francesca Healy Cottage, part of St Vincent's Hospital Melbourne's Health Care for the Homeless program. The Cottage is a unique five-bed health facility providing care within a welcoming, home-like environment for people experiencing homelessness who may have complex physical, social and psychosocial needs. As a Health Care Worker, you will help create a safe and welcoming environment, build positive and respectful relationships with clients and provide practical, personal and health-related support. Working alongside a multidisciplinary team, you will support clients with their daily needs while promoting dignity, independence and wellbeing. This is a rewarding opportunity for someone who is passionate about working with vulnerable members of the community and delivering compassionate, person-centred care.

Classification: IN39 $37 per hour plus a casual loading fee of 25%

What You Will Do

As a Health Care Worker, you will:

Create a safe, welcoming and respectful environment for clients

Build positive therapeutic relationships with people from diverse backgrounds and with complex needs

Provide personal care and assistance with activities of daily living (ADLs)

Support clients with diabetes, wound and continence monitoring within your scope of practice

Provide medication assistance in accordance with relevant policies, procedures and scope of practice

Prepare and provide simple meals and nutritional support

Complete housekeeping and cleaning activities while maintaining appropriate infection prevention and control standards

Complete accurate documentation and participate in clinical handover each shift

Undertake administrative activities including answering telephone calls and emails and liaising with internal and external services

Maintain required training and competencies

Attend and actively contribute to regular team meetings

Work collaboratively with colleagues and the broader multidisciplinary team to support coordinated, person-centred care

About You

You are a compassionate and reliable Health Care Worker who is comfortable supporting people with complex health and psychosocial needs. You can work independently when required while contributing positively to a multidisciplinary team.

You will bring:

A nationally accredited qualification at Australian Qualifications Framework (AQF) Level 4, or be currently working towards completion

A minimum of two years' experience as a Health Care Worker

Excellent interpersonal skills and the ability to communicate respectfully and effectively with a diverse client group

The ability to confidently and appropriately respond to challenging or complex behaviours

Strong written and verbal communication skills

The ability to work effectively both independently and collaboratively within a team

A commitment to person-centred care and the mission and values of Sister Francesca Healy Cottage and St Vincent's Hospital Melbourne

Willingness and availability to work sleepover shifts from 4:00pm-7:20am

A current First Aid Certificate, or willingness and ability to obtain one

About Sister Francesca Healy Cottage

Sister Francesca Healy Cottage forms part of St Vincent's Hospital Melbourne's Health Care for the Homeless program, which supports people who are experiencing, or at risk of, homelessness. The program brings together services across St Vincent's to provide better integrated and coordinated care while developing responsive models of care that meet the needs of people experiencing homelessness and complex health and psychosocial challenges. The Cottage also sits within the Health Independence Program (HIP), which aims to improve outcomes for clients through integrated and coordinated care across hospital and community settings. HIP supports hospital demand management and patient flow while reducing duplication and fragmentation of services and promoting person-centred care planning.

Sister Francesca Healy Cottage is a five-bed health facility located within a home-like terrace house setting in Fitzroy. The service supports people with complex needs that may include homelessness, substance use, mental health challenges, disability and family violence. Working as part of a multidisciplinary team, Cottage staff provide physical, social and emotional support within an environment centred on safety, dignity, respect and compassionate care.
