About the role
The Clinical Nurse Specialist - Wellbeing & Health In-Reach Coordinator is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessment and referral to services of school aged children and young people.
Working closely with education professionals in learning and support and leadership teams within schools, you will help coordinate healthcare needs for children and young people, to optimise health outcomes.
You will be involved in coordinating care in Walgett High School, Walgett Primary School, Goodooga Central School, Lightning Ridge Central School and Collarenebri Central School.
This role sits within the Service Delivery Directorate - Child, Youth & Families Division, and is supported districtwide by likeminded and experienced professionals.
