Care Compass Collective is a Queensland based NDIS provider delivering psychosocial and mental health focused disability support. Our support is relationship led, psychology informed and shaped around each person's goals, needs and everyday life. We deliver in-home support, community participation, mentoring, employment assistance, psychosocial recovery coaching and short-term respite across South East Queensland.
ABOUT THE ROLE
The Care Coordinator oversees a caseload of participants within their zone, from first enquiry through onboarding and ongoing support. You will report to the Director and work closely with our Support Workers on rostering, handover and participant matters. This is an internal CCC coordination role, distinct from NDIS funded Support Coordination.
The role is based at our Slacks Creek office, with regular travel across our service zones and work from home by agreement.
WHAT YOU WILL DO
Manage new participant enquiries from first contact through to onboarding, including intake and orientation
Assess whether CCC can safely meet the needs of a new referral and make a recommendation to the Director
Draft individualised participant support plans in line with each person's NDIS plan and goals
Recommend matches between participants and Support Workers, and coordinate rosters for your caseload
Monitor documentation and service delivery standards, including shift notes and participant records in ShiftCare
Prepare documentation ahead of NDIS plan review meetings, including progress summaries and evidence of support delivered
Maintain regular contact with participants and their families, carers and friends
Manage referrals and build relationships with allied health professionals, support coordinators and plan managers, including representing CCC at expos and industry events
Report hazards, incidents and near misses in line with CCC procedures
WHO WE ARE LOOKING FOR
Organised and reliable, confident managing a caseload independently and prioritising competing demands
A strong relationship builder, comfortable with participants, families and external professionals
Sound judgement, with the ability to identify when a matter needs to be escalated
Calm and adaptable under pressure
HOURS
Around 30 hours per week, Monday to Friday, generally 8am to 4pm. Some flexibility is needed for participant meetings and community events.
WHAT YOU NEED
Demonstrated experience in disability, mental health or psychosocial support coordination, or a related caseload management role
A relevant certificate with relevant experience, or equivalent experience
Excellent written and verbal communication skills for participant documentation and professional correspondence
Confident with technology and quick to pick up new systems
Comfortable working on an Apple Mac, or willing to learn
Current NDIS Worker Screening Check, or willingness to obtain one
Current Working with Children Check (Blue Card)
Current First Aid and CPR certification
Completion of the NDIS Worker Orientation Module, or willingness to complete it before commencing
Current driver licence and access to a reliable vehicle insured for business use
Direct psychosocial support experience and a Certificate IV in Mental Health are well regarded.
